What is the High Blood Pressure Clinic?
Appletree’s High Blood Pressure Program provides in-person care and ongoing monitoring at multiple Appletree clinics to help patients reach and maintain their target blood pressure.
How can the program help?
Through regular follow-up appointments, our physicians monitor your blood pressure, review your progress and adjust your treatment plan when appropriate. The program provides ongoing support to help manage high blood pressure and reduce the risk of related health complications.
How Can I Access This Service?
To access our hematology services, you must have a referral from a family physician.
